Request for Funds

Please use the attached forms when requesting funding from the SWC Foundation. Form A should be used for campus related activities and Form B should be used if you are requesting the Foundation sponsor a community event.

• FORM A - Campus Activity/Organization (91KB, PDF)
• FORM B - Community Event Sponsorship/Advertising (95KB, PDF)

Please ensure that you complete/submit the Post Project Evaluation form (third page of each of the above attachments). Not completing it could possibly jeapordize future funding requests.
